Popup window with Input Field

I'm trying to make a pop up window like tooltip but has input fields inside it like a normal modal window. Like tooltip below, but having input fields like the modal window images

Unfortunately, this is not fulfilling the requirement.

I also tried some plugin from https://www.jqueryscript.net/tags.php?/popup%20menu/ , but nothing can have input field like modal window.

Another thing I tried is using jQuery mobile. Unfortunately, the jQuery mobile forces me to use a specific range of jQuery version. While the project use the jQuery v3.x from the beginning. So, if I change the jQuery to v3.x some features start to behave strangely. More importantly, the code are made by team, so I cannot just change the jQuery version just like that.

I need suggestion from experienced from web developer or other developer, who might be encountered or done the same thing, in the past. It would be best if such a plugin are already available.
