How does a web application serving different companies

I'm trying to understand how does a enterprise level web-application serves different companies since each company has different amount of data.

Does the web-application create separate database instance for every company?

For example, company A and company B both using the same service from a web-application. How does the web-application distinguish between them and connecting to the correct database when users from company A and B log in?
