How to write an abuse/hacking attempt report mail?

I am wondering whether there is any official template or anything like that I can use to report a hacking attempt on my website (against Apache). I would like to let the provider know that there was a hacking attempt from an IP address of his range.
Do you know whether there exists anything like that or do you know how to write such a mail?
