upload image and tow text to Mysql

I want to upload image and tow text(movie_name- movie_image) into MySQL as Shown in the attached picture by php fails throw the Browser AND and stor image in folder , naw I am entering data manually but I want to entering Image url and texts by php >> I do not have knowledge of web design or PHP >> thank to all
