Creating a gateway to deliver a coupon code and a URL containing a random number

I'm looking to piece together a simple gateway that will take a volunteer's:

1. Name
2. Email
3. Product they'd like to receive (optional)

...and then after submitting, provide them with:

1. A unique coupon code for the product (can be stored in a spreadsheet)
2. Link to product, where 4 digits at the end of the URL must be randomly generated for each user (this could be a number stored next to the above coupon code in the spreadsheet too).

With minimal programming experience, what's a good angle to take on this?
