I'm seeking for ones special free automation-formbuilder cms/script :

for example something with the pannel that user would enter on , with the programmed forms we entered it's details before , and would save it's information and returns the list on the special period ( example end of the every month) . I wanna some prompts about the forms I've entered before.

did anyone use something like that ?
