Which 3D web framework should I use for developing 3D medical imaging S/W?

I am looking for 3D Web framework to study for developing 3D medical imaging tool.

1. View 3D medical images with DICOM, such as MRIs, CTs, and so on.
2. Annotate(label) on the 3D images through Web.

I found some: three.js(AMI), vtk.js and X3DOM, but don't know what to use.

Based on that I know little about 3D graphics(actually now studying through VTK(not vtk.js) ), can you guys recommend which frameworks to use regarding the present and future usage?
