Web gui for completing actions in bash, with workflows

I'm looking for a web gui that can be used to allow user input and complete functions in bash based on form input. Basically run a bash script based on form input.

I would also like it to have its own system for keeping track of items/requests allowing for approvals and workflows. Hoping to take the original information inputted and then send this to the bash script after it has been approved. This is optional.
