Using custom forms to build paragraph/letter.. best way?

Not exactly sure how to ask this.. but basically looking to make forms within forms. Imagine lists, radio buttons, checkboxes, etc etc... Where all the values from each input will help generate a written letter/form.

Basically looking to make entry at work much easier. Often times find myself writing the same things over and over. So just basically wanted to build a "letter-maker". I would answer a few of the questions in these webforms and it would output a "paragraph" based on my answers in a pdf/web doc.

Any ideas where I should start or if any GUI editors that can help with this?
