Does games and video call services like Omegel directly connect two hosts?

I know that we can pull someones ip in many multiplayer games and on omegle useing wireshark but how?? Doesn't all host connect to game or omegle server but if that was true we should get the servers ip instead of the computer on the other end??. Does the both host directly connect with each other?? Sorry for asking if you think the question is foolish it just came in my mind and I can't find an answer.
