Wanted: Programming language for surveys and other forms

I am looking for a software that allows forms like class tests and surveys to be taken, as such there could be a printed version as a handout (TeX preferred) same as a web online version on a central server.

Since the structure might get a little complex (some answers excluding others or let questions be skipped) I imagine something like a programming language specifically designed for such a task. I remember, in the late 80s, there has been such, but it seems to have been completely disappeared since then.

Any hints, ideas?
