Best method: business questionnaire/analysis web or app?

My question is pretty general since the only programming language I'm familiar with is HTML/CSS and a bit of PHP, and I'm not really sure where to start.

Anyway, I'm looking to build some kind of web page or app where clients can fill out a form or questionnaire, with questions/answers ranging from simple drop-down/check box selections to a bit more complex 'if answer yes -> show a and do this, else -> show b and do that' kind of stuff. The goal is for the client to get a score 0-100 and different information/results depending on if the score was below 50, above 90 etc. There is also a need for an easy-to-use interface, responsiveness, and a way to somehow save the results, get a link with the results that can be shared with others, etc.

Could this be accomplished through a responsive web page with a PHP questionnaire, or would that be stupid?
