How can i make simple telemedicine app/web?

i am An. I am having a small project for myself that connecting patient and doctor. First of all, I just need it has a simple function as:

• there is a form, patient will fill the form and then send to doctor. After that doctor will preview it then make an appointment for chatting( by text) with patient.
• i do not know anything about coding. Could you guys please give me a recommendation or solution for it?
• Can I use google form?
