dynamic checkbox list using php, html, css

I need to make a list of check-boxes, like you would find in your Email inbox: Dynamic size and the info displayed is brought from the database. this is an example of what i need I am using html, PHP, and CSS. Please excuse my ignorance if this is an obvious question. I have tried searching for it but i didn't know what to write as a question(why I am using a picture) because my knowledge is unfortunately very limited in web development. thanks.
