Is there a way to extract information from a fax document and place it inside a website form

I have 87 hard copies that each have 3 people's names and dates of birth. I was wondering if there is a way I can extract these two values from the paper and place them on a webform autonomously. As opposed to manually inputting them on the webform. Thank you in advance.
