Website Description not correct on Search Engines

I am trying to figure out where I have gone wrong. While I was developing the website google crawled through and picked up on not really finished version of the titles and descriptions. Now when I go to search the website on google, it has the wrong description and in one case, it even has wrong title.

I have now made sure to include the <title>MyTitle</title> tag on every page and <meta name="Description"/> Then I have gone to http://ift.tt/IUXtiv and requested the website to be re-indexed. It said that it completed successfully, however when I search now I still see the old content.
