How to specifically get these HTML Glider effects and contact form?

Going through stackoverflow I can't seem to find a request similar or one that will benefit me! I keep finding online, slideshow gliders and basically slideshow effects,I cant find the effect I am looking for. I tried stackoverflow many of time using different wording like, Gliders, scroll glider, and moving images. I could not find anything relevant for what I am looking for. But i'm trying to build a website and I have no idea how to do these Pretty cool effects!

Now I have my Code that I am using and need specific help to this so no one else has the same request but I also have a request form that has a few issues! Issues:

1. Emailing Status does not work

I believe this is useful not for me but anyone that is either trying to fix their own contact form/who are having the same issue and people who want to add the cool glider effect I am looking for!

Hopefully someone can tell why their is a issue with this. To get a better understanding with this request here is a image I have supplied! Thanks in advanced.
