In PHP and SQL, how is a Terms and Conditions agreement handled from a developer standpoint

When you click "Agree" Is it just a variable that is passed to a sql column in some database or is it something more complex?

Is it treated different from say a blog to a subscription service that will charge you say $10 a month like Netflix?
