How do I go about creating a desktop application that will prompt for a 4-5 digit number and it will open up a web browser to a specific URL?

I am trying to create a desktop application that I can click on and it will prompt me to type in a 4-5 digit number (for ex: 12345). If the number does not exist then it would say not found. It will then open the default web browser to a specific URL. The URL for example would be https://www.google.com and the number entered would go at the end of the URL: https://www.google.com/12345. This would be used to make reports easier to locate in one spot instead of searching for them each time. I need to know the best and simplest way to go about this and maybe even something that can be customized now or in the future(make it look better). I have tried researching some ideas but no luck. Please help me with some ideas! Please be specific and where I might find some additional help/resources.
