What is the best way to create a yes/no application from a workflow (like a wizard)?

I have drawn a workflow to imagine my future application. Now I am searching for the best way to convert this workflow into an application (windows first, android perhaps, web after, ... but it's not so important I think).

Imagine a workflow to find the best way to wash your clothes and then the questions :

• Is it color => No => Is it white => yes => is it coton? ... and so on
• Is it color => Yes => Is it whool => ...

So all the answers are only yes or no...

And if no final result the possibility the restart.

For a console application I know how to do this with a lot of if then else. But I want a form application. And I don't know what to use (in C# or java or ...).

Could you help to find an example? Or put me on the best way to program this?
