Google MyMaps Input Form

I'm building a web app and I need help with creating an input form for Google My Maps.

The form will allow users to enter a location of a fruit tree they will find.

Once users enter the details (shown below). The location will be automatically added to Google My Maps. The locations must also use my custom icons automatically. e.g. Orange Icon for orange tree

The fields will be: Address City Town Postcode (Required) Fruit (Dropdown) Description Box Image (Users will upload an image)
