What's the best library to make a survey page from scratch?

I want to make a survey page that includes logins, questions with drop downs, results page with pie/bar charts, etc.

I don't want to use existing survey websites and make my own. I can definitely make all this using only html, but I know there are libraries that will help me make it look professional.

What are some libraries I should take a look at?
