HTML: Adding icon image to Title Bar

I know this question is already asked so many times. But for some reason I can't add an icon image inside the title bar for my website, I've already tried this: <link rel="shortcut icon" type ="image/x-icon" href="location/image.ico">

I put that code inside HTML Head, below the title tag, but it still not working. Did I make a silly mistake, or what? The image I used has size of 16x16 pixel, and .ico format.
