web: Create a table from an html form

I want to build a data visualization web app. Let's say I want to insert bodyweight. When I insert the bodyweight I want to create a table that shows the last 5 data inserted.

How can I do that?

basically the user sees the input for the new data and the last 5 data already inserted.
