What are different ways in which a Web Server may be made to fail

I am looking for different ways or should I say Goof Ups, Deliberate take downs or Hostile take downs that the community here have witnessed in their experience in which a server has been brought down.

This could be as small as pulling the plug or a DDOS attack. What all have you experienced ?
