How can I add a card with preset design but individual information (like a telephone book) automatically

i do not really know how to accruatly describe my problem but i am looking for a contraption similar to eaby, amazone etc. The customer can fill out a formular and their information will get added to a list of containers with a preset design. (ebay: customers addet product ends up on a list where everyone can find it).

I hope it is understanable what i mean and i would be glad if someone could tell me what this system is called or how it is made (what programming language you would need etc)
