Tool for reviewing live web pages?

I'm looking for a tool to embed in my webpage (or maybe a browser plugin) that does the following:

1. Allows reviewers to report issues, request features, and provide feedback while on the website.
2. Provides an image or some indication of the part of the website they are providing feedback on. It would be very effective if they could highlight a dom element, comment on it, and then submit that picture.
3. Creates a new issue in GitLab/GitHub when the user submits the feedback.

I've searched for a while, but it seems bizarre that I'm not finding anything.
