How to add chat box as a icon to the website?

I need an Icon in the corner of a website which will appear after 5 seconds and when we click that icon It should open Email Us with fields Name,Email and question to be asked. And send email button should send the query to administrator email ID. How can I write this? Any link or some example will be more beneficial.
