How To Log User Information With Web Beacon / Pixel Tracker

I have a client that wants to track their customers responses with an email survey. There are three main criteria that the client needs to store:

Customer email address. Customer satisfaction level. Customer referral source.

The client will be sending out the emails manually so the customer will not be accessing any pages on the clients website directly.

Does anybody know how this can be done?
