auto fill html form using php from excel

For Examle: I have a HTML form which has 2 text input fields: id and name. My excel sheet table has same named columns and the values are id=1, name=mark. Now, if I enter '1' in my HTML form's ID field & press enter and the name should be filled out according to my excel sheet. simply like v lookup in excel.

please help me to solve this
