How to add password with Web2Disk

I want to get all the pages of a web site with the application Web2Disk. The problem is that I don't get all the pages. The website is made with java web. I think that I have some problem to connect the user when I run the app.

I created the excluded page for the logout so this part is ok. But in the Passwords and Forms configuration, I add my URL but I don't know what to write in the "Form Data" area.

Can someone tell me what should I write here and if I missed some settings please ?
